If you do not submit the form we cannot send your scholarship checks to your college/university. 

Additionally, please use the Scholarship Contact List to mail your thank you letters to your scholarship donor. If you are not sure who your scholarship belongs to please check your school email, Miss Kristy sent the list to you.
